Applewild School Required Admission Forms

Each student is required to submit an Applewild School Application Form and the following recommendations:

* Applying for Kindergarten or 1st grade:
   Kindergarten or 1st grade recommendation form

* Applying for grades 2 or 3:
   Homeroom and additional teacher recommendation forms.

* Applying for grades 4 through 6:
   Homeroom teacher, additional teacher recommendation forms, and personal recommendation form (optional).

* Applying for grades 7 or 8:
   English Teacher, math teacher recommendation forms, and personal recommendation form (optional).

Applewild School Admission Forms

Admission Application: Required form to be competed by parent(s).
Kindergarten & Grade One Recommendation Form: Required form to be filled out by your child's current Preschool or Kindergarten Teacher.
Grades 2 - 6 Homeroom Teacher Recommendation Form: Required form to be completed by your child's current homeroom teacher.
Grades 2 - 6 Additional Teacher Recommendation Form: Required form to be completed by any of your child's teachers other than homeroom. 
Grades 4 - 8 Personal Recommendation Form (Optional): Optional form that is filled out by an individual that has known your child for a period of time. (girl/boy scouts leader, karate instructor, dance instructor, extra-curricular activities instructors)
Grades 7 - 8 Math Teacher Recommendation Form: Required form to be filled out by your child's current math teacher.
Grades 7 - 8 English Teacher Recommendation Form: Required form to be filled out by your child's current english teacher.
AppleCore Scholarship Application: Required application to be filled out, along with the Applewild admission form, for any student wishing to apply for the AppleCore scholarship.
International Admission Application: Required application for international students applying to Applewild School.
