CHECKLIST TO COMPLETE YOUR APPLICATION FOR FINANCIAL AID

Items that need to be sent to School and Student Service (SSS) for Financial Aid in Randloph, MA:

• A copy of your Parents Financial Statement (PFS) must be completed and mailed with the appropriate filing fee ($35 online) ($47 hard copy) to the School and Student Service for Financial Aid (SSS), by February 3, 2012.
• If you are separated or divorced your former spouse must also complete a PFS and send it to the SSS, with the appropriate filing fee, by the deadline.
• A signed copy of your 2011 IRS 1040 and all supporting schedules.
• A copy of your 2011 W-2 forms and any 1098, 1099 forms.
• If you own your own business, a Business and Farm Statement must be filled out.

Items that need to be sent to the Applewild School Financial Aid Office:

• A copy of the PFS form that you sent to the SSS.
• Form 4506-T must be signed and not dated to obtain your official filing information from the IRS, if necessary.
• A signed copy of your 2011 IRS 1040 and all supporting schedules.
• A copy of your 2011 W-2 forms and any 1098, 1099 forms.
• If you are separated or divorced your former spouse must complete a separate PFS and send a copy to Applewild with all supporting documents.
• If you are a first time applicant for financial aid, you must also submit a signed copy of your 2010 IRS 1040 and all supporting schedules.
• If you own your own business, a Business and Farm Statement must be filled out.
• A check for the $35 filing fee, payable to Applewild School.
