6. Financial Aid Checklist

If submitting your PFS Online, after submission you will be prompted to print out a cover sheet that is pre-populated with your information; you are instructed to use that cover sheet when mailing in required documents to SSS. If you wish to submit additional documentation as part of your application, you can use the cover sheet loaded here on this page; you will need to fill in your information and PFS ID number to ensure that the documents you send are properly identified with your application.

If submitting your PFS in print, you should use a cover sheet when mailing in documents required by the schools to which they have applied.

Note: Families who wish to request that their reports of family contribution go to additional schools (beyond the six schools they indicated in their PFS -- submitted online or in paper) should make such a request on the PFS Online. There is no charge for these additional requests.

Click on the PFS logo below to begin your online Parents' Financial Aid Statement (PFS). You only have to complete one PFS even if you have several children applying to our school.  You will be given a password that will allow you to return to your PFS at a later date before submitting it. You will be given instructions about submitting required backup documents by mail. Your documents will be handled with the utmost level of security.  Mail the required documents to: SSS by NAIS, Application Processing Center, P.O. Box 449, Randolph, MA 02368-0449.
