Register for a summer at Applewild!

1. Download and print one registration/medical form for each camper registering.

REGISTRATION & MEDICAL FORMS (all campers)

EMERGENCY CONTACT FORM - Required for all campers

2. Complete the form. Use the course descriptions on this website to help choose activities on the correct session activity options form.
3. Print out session 1, 2, or 3 and complete form.
4. Mail your registration form and health form along with a $150 deposit to:

CIT Letter (Required to register for the CIT program): There will be a $29 fee for the CPR/First Aid course. Please include this payment with your deposit.

Writing Institute Survey (Required to register for the Writing Program)

**Flex Scheduling**

We are offering flexible scheduling this year. You may sign your child up for one week of any of our Summer Adventure Camp sessions and attend the second week during a different session. Email Milissa Cafarella at mcafarella@applewild.org for more details.
